If you have had these symptoms for a while, it's time to see a doctor. If these symptoms are acutely occurring (only one time) after a difficult bowel movement, you need to take care the constipation first by increased exercise & fluid intake, eating high fiber foods or taking stool softener +/- laxative (miralax, milk of magnesia) and observe other symptoms. See doctor if symptoms persist.
