Fenofibrate can increase creatinine and cause acute kidney injury, usually with rhabdomyolysis in some patients. This is reversible with discontinuations. The mechanism is unknown, but seen mostly in high doses, older patients, and preexisting kidney issues with diabetes. See PubMed references 17368275 and 28775808
