She may have severe fecal impaction or a quite big, invading rectal tumor. The latter is more likely. Your sister has to suffer from severe constipation with either condition. Good luck to her!

It is a occasionally impossible to pass the scope beyond the rectum in people that have tortuosity or sharp turns in the colon. These do not usually cause symptoms, but just make it difficult to pass the scope throughout the colon. Good luck.
