If the diaphragm is elevated, it usually means it is paralyzed and it is being sucked into the lung cavity, hence it is elevated. If it is paralyzed, the phrenic nerve is the culprit since this controls the diaphragm. Sometimes ultrasound of the diaphragm can confirm if it is moving at all or if it is paralyzed or weak. This can cause shortness of breath so get the ultrasound done for diagnosis.
