Can all types of angina, including microvascular angina and vesospasmic angina, be reliably captured with ecg changes on treadmill stress test? what is the diagnostic specificity and sensitivity of stress echo for different types of angina?

Angina is chest/arm/jaw pain on exercise that stops when you stop. There is no way to capture except by a good history and physical. Treadmill may show changes with exercise due to not enough blood getting to heart area supplied by a narrowed/blocked artery includes vasospastic if you "spas" on the treadmill.

Patients with microvascular angina usually have normal stress tests. Coronary vasospasm may not occur during a stress test. In my opinion, a nuclear stress test is more accurate than a stress echo in diagnosing significant coronary artery blockages. Both tests may miss blockages less than 60%. If we think a stress test is a false positive, a CT angiogram is helpful in measuring amt of any blockage
