One needs a better handle on the Back pain. Is it Nerve Entrapment due to pinched nerve? Muscle Spasm or Bone Degeneration/ Arthritis or Injury? The best management is usually based on knowing the underlying cause. A visit with the PCP is where to start. For Crohn's disease one should stay close to the Gastroenterologist care.

Just from this description alone, it is impossible to differentiate back related causes of your back pain from potential causes related to your Crohn’s disease. At a minimum, you should return to your gastroenterologist for follow-up of your Crohn’s. Good luck.
