See a dermatologist or podiatrist for the correct diagnosis first. If problem is mild, you may not need Tx & you will be instructed how to control the spreading and relapse. If your toenails are severely affected, you will need to be treated with oral medication +/- topical medication. When treated, it will take months for a complete recovery and medication' s adverse effects will be monitored.

You are susceptible to fungus infections. Your immune system could be weakened thus unable to control the infection. This will be an on going condition . If your feet perspire use an anti-perspirant spay on your feet. Oral medication for active infection is most effective. I would use a topical as well as a preventive. Clean and/or change your shoes. It will not be easy. See PCP or podiatrist

It might depend whether you have 1 specific toenail or multiple nails involved. Sometimes the problem is not an fungus, but some other dx such as psoriasis, lichen planus, or even a more serious entity. Suggest you see a provider skilled in performing a nail clipping for histological testing and culture if necessary for an accurate dx. You will be treated properly if fungus or something else.
