Could be a number of possibilities but most likely some fungal or bacterial infection. A warm Epson salt foot soak might ease the pain but best to see the PCP or Podiatrist (Foot Doctor) soonest for evaluation and appropriate management.

You can see and touch bone. Go to hospital immediately Exposed bone makes you Vulnerable For osteomyelitis bone infection. You can loose an extremity if not careful.
