The stress test will likely provide the answer as to whether your pain is heart-related, assuming you can exercise to an extent that will increase your heart rate. It will be especially helpful if your pain occurs during the test. Anxiety can cause VERY intense chest pain so the intensity of the pain is not enough to figure out the etiology of your pain. More description of your chest discomfort would be helpful in giving you a differential diagnosis. While a treadmill stress test would rule out coronary artery blockages as long as you achieve your target heart rate & have no ischemic EKG changes or characteristic symptoms of angina, it won't exclude coronary vasospasm as a cause of chest pains. Being diabetic, it's important to see a cardiologist
