Titrating lamictal by 25mg a week for seizures. will be at 400mg total daily in a few days. been on lamictal for 2 years. have a red flat rash on chest size of thumb 2 days. is it possible to develop the steven johnson rash at this point?

A tiny focal rash on your chest does not represent a presentation of Stevens-Johnson Syndrome, and very doubtful that Lamictal is causative. See a dermatologist if this progresses or expands.
