A RN, PA, and NP all must enter codes for the treatment they provide to patients. The consulting physician reviews them but state laws dictate all protocol and procedures.

No, it is the provider usually with the assistance of software many times, nurses are not usually involved in coding. Many facilities have dedicated coders/billers that can further get involved.
