Spinal TB got to the spine through the blood. It gets into the blood after first being part of a lung infection and that infection can be active or dormant. The germ grows slowly and transfer is likely to people who live with the person for months. It is not passed easily like strep or a virus. A casual conversation & meal are not likely ways to become infected.
