Sometimes while doing something, if i feel that something has come out of the penis, then later i can see that a little urine has accumulated in the head of the penis. what is the reason for this and how to get rid of it?

Make sure you get all urine out after you think you are finished. Do not hold urine for long when you have the feeling to go. drink moderate amt of fluids including water. It is probably some urine left over after you void. It could be due to other reasons too but that would require an exam. Try that. If not better pl see a Urologist.
