Estrogen does not affect PKD by my knowledge but I think the reason your nephrologist prefers a lower estrogen dose because estrogen may cause or worsen hypertension and it may increase size of liver cysts which are common in PKD. Besides that, a lower estrogen dose in OC pill does not change the effectiveness, i.e. prevent ovulation, which is mainly dependent on the progestin component.
