In view of the CAC score you have some arteriosclerosis but we don't know about atherosclerosis or mild plaque within the lumen of coronary arteries. Was your target heart rate reached during the stress test? Did you have any chest discomfort then or suspicious symptoms during other exertion or with emotional stress? If so, a coronary angiogram or a CT coronary angiogram would be the next step.
