SIBO is not a likely cause of elevated fecal calprotectin, especially at such a high level. Same for celiac disease (your other question). At this high level, the most likely explanation is inflammatory bowel disease (Crohn's, ulcerative colitis) that were missed by colonoscopy, such as Crohn's of the small intestine.
