What is the worst possible outcome if i get water in my ear when i have a middle ear infection and my eardrum is perforated? and how likely is this outcome? what symptoms would i have before this outcome?

It depends on the situation. The water can transport any germs that travel in the water or lay inside the canal inside the sensitive membranes of the middle ear space and trigger infection. Pain and drainage would eventually signal the process is there. The potential is there but I know of no studies that note the document risk.
