Dienogest is a synthetic progestin, which can cause spotting or mild bleeding ("brown discharge"). Any kind of bleeding or spotting usually means an activation of endometriosis implants in the abdomen, causing pain accordingly. Oral contraceptives are commonly used with endometriosis, as oral bcp's suppress ovarian sources of hormones, reducing symptoms of endometriosis.
