These are characteristics of occipital nerve impingement as it exits neck vertebrae. History of whiplash? Osteoarthritic spurs or very tense neck muscles could be the culprit. An xray of the neck is worthwhile. Intermittent heat, soft neck collar, anti-inflammatory meds like ibuprofen or naproxen,physical therapy should help. See your MD if not improved with these measures.

Your description is classical for either Greater or Lesser Occipital Neuralgia, and a visit to your doctor should be able to readily confirm. Occipital nerve blocks could likely reverse much or most of the symptomatology.
