This is far from my field of expertise, but 46 strikes me as well below range of normal. I assume your 56yo friend is under a Physician's care as she is monitoring glucose level. Please have her call her GP, Internist, Endocrinologist and report ther reading. It may be an outlier, but should be evaluated by her Physician.
