You may have an anal fissure. However persistent throbbing pain raises suspicion of a thrombosed hemorrhoid or a perianal abscess. Both conditions are painful and require accurate diagnosis and appropriate treatment. I favor your being evaluated by a proctologist. or a colorectal surgeon for best care and follow-up. In the meantime apply warm compresses.
