You may have a problem with your tibialis anterior muscle (Google it). Can you walk on you heels with both feet. If not may be tibialis anterior weakness. Show to Dr.

This is not an unusual problem. It is seen in a few patients but not usually symptomatic. That is one reason the toe box size and depth are important. Over active tendon, extensor hallux longus . An adjustment to the orthotic might help. I have never treated anyone with this as being pathological.
