Do you need to "feel" capsaicin for it to be working? that is, as you get acclimated to the burning etc of a given concentration of capsaicin, do you need to increase it for it to keep depleting substance p?

It is possible and that's why there are different concentration; however, the mainstay of treatment is prolonged use. It may take up to 2 weeks of using this medicine regularly before your symptoms improve. You may not need to feel to get its pain relief effect in the long term.
