Rectal spasms or levator muscle irritability may be part of the problem but wouldn't explain itchy urethra or testicular pain. I recommend having your physician or a urologist inspect the anus & perform a rectal exam,evaluate the prostate and examine the testicles & then advise you.

Your symptoms do not indicate anything serious since all tests are neg and I hope also had a physical including the prostate. If so try not to concentrate on your symptoms and wear a support underwear, eat healthy, exercise, keep busy and if you want to- do relaxation/meditation so you do not worry about all the symptoms . do not self diagnose. I think you will be fine
