Any family history of Gilbert's syndrome or hemochromatosis? I think more labwork is needed here. I recommend consulting a hematologist and/or a gastroenterologist to direct the workup & follow-up. Iron overload damage to the liver takes years but I still would start seeing a specialist at this time.

With + IgG P41 Ab, he was exposed to Borrelia which causes Lyme and I wonder if his doctor ordered this test because of the migraine history. Total bili is high and if it is mainly indirect bili, the cause is likely Gilbert's syndrome which affect 10% of American population, a benign and hereditary condition. Slightly low ferritin alone with high normal iron level are not enough info for advice.
