PCOS essentially is responsible for irregular or absent ovulation, but the level of estrogen produced is higher for longer, but does fluctuate, so depending on exactly when the estrogen level is tested, it may on some occasions be relatively low, and bleeding from an overstimulated endometrial lining will result as a withdrawal symptom, which is usually not a post-ovulatory event.
