Ketones appear in the urine anytime the body breaks down fat cells to create glucose. This can happen if you are fasting for any period of time and at low levels is not a problem. Odor in urine can be created by metabolic waste, off gases from bacterial action or byproducts of some foods (like asparagus) .

Ketones may be present when urine is tested during fasting with concentrated urine (strong odor is due to high urea content with concentrated urine, ketones give a sweet odor). You just need to increase fluid intake about 30%. High ketones in urine for a diabetic patient is a serious condition, not in your case. Urine ketones can be high in people with low carbohydrate, high protein diet too.
