By far the most common cause of strong vaginal odor is bacterial vaginosis, an overgrowth of mostly normal vaginal bacteria. The main treatment is the antibiotic metronidazole by mouth. BV can be triggered by STIs if at risk (e.g. gonorrhea, chlamydia) and the STI trichomonas causes similar vaginal discharge and odor.
