I think it is Morgellons disease a dermatopathy. It is still unclear if it has a biologic basis of is a psychiatric entity with delusions. Morgellons is a rather controversial skin condition, treatment can only be based on actual findings and seeing a dermatologist for a possible biopsy of the lesion would be best.
