None of the issues you mention are any sign of a health issue, i.e. you are normal. For healthy diet: http://www.choosemyplate.gov/., exercise 30 minutes/day, drink enough water daily so your urine is mostly colorless, have safe sex, no tobacco alcohol weed or street drugs. Your brief description probably does not raise red flags. But it's time for you to see a General Practitioner for a complete physical exam, medical and medication history, baseline lab tests, vaccination history, and possible imaging. All of the above are required for a Physician to asses your overall health. Your verbal snapshots are far from being enough information.
