You should wait a week with absolutely no symptoms before working out. Start slowly and gradually increase activities over the next couple of weeks as your conditioning improves.

When you feel like it. If you had a minimal case you never need to stop. If you had lung/respiratory damage it will depend on how fast you heal, the more intense the longer it takes. Listen to your body. It will tell you when you are pushing to the limit and then you back off.
