Liver failure: With liver failure, you develop many physiologic derangements. You lose protein and muscle mass so arms and legs get thin, but ascites can also develop which gives you a large belly full of fluid. You generally don't see liver failure patients with skinny bellies, unless it is a very acute episode of hepatic failure.
