Yes, with aging scrotum can sag but you only need underwear with scrotal support if the sagging causes discomfort.

Wearing scrotal support underwear is a personal choice and is not a necessity. However it is helpful for physically active people and sportsmen.. Yes the cremasteric muscle does loose some of its strength with age and may cause the testes to hang lower but at what age varies with the individual.
