There is minimal t2 signal alteration adjacent to the tips of the posterior horns of the lateral ventricles on either side. no other focal lesion. what does this mean?

MRI finding has to be correlating with clinical symptoms and signs, especially there is only a minimalT2 signal alteration. Please discuss with the doctor who ordered the MRI.

This has to be clinically correlated, and cannot do so without additional information. Would suggest you contact the ordering doctor and the radiologist and ask you question to those who know you best.
