Most cancer's are believed to take some time for normal cells to "change" enough to become cancerous. And often there are changes that we may refer to as metaplasia and/or dysplasia... and eventually those cells may mutate further and become cancerous. I'm cautious about using the words "always" or "never", as we learned, there are exceptions to most rules and nature often does what it wants.
