How do i get medical paperwork stating i am quarantined due to covid exposure for proof of leave at work?

We are all or have been exposed to COVID this past two years. You need a positive PCR test from the people who you believe you were near that tested positive and show the results to your HR person at work. Who quarantined you?, if self need proof of positive exposure.
