Health anxiety is most often a socially convenient coverup for the true nature of ones emotional triggers. People can accept/give comfort to those with health concerns easier than those just freaking out over lifes' stressors or covert triggers. Anyone can have a hidden illness but those who let the idea control their life need help learning to manage stress.

It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water. Wish you good health!

The younger you are, the less likelihood of heart disease. Severe panic and anxiety can make everything worse and they can make you feel having a heart attack feel so real. If work-up all negative already, I would recommend working with your doc/psychiatrist to get your anxiety/panic under better control. There are MANY options. Lexapro/Ativan are good but not for everyone. Good luck.
