If the PVC's have a LBBB pattern they are emanating from the dilated right ventricle. This may be potentially dangerous arrythmogenic right ventricular cardiomyopathy. Even though you're not experiencing many PVC's, multiform PVC's are considered more dangerous than uniform PVC's. I strongly recommend that you be evaluated & followed by a cardiologist electrophysiologist for best advice &care

Not sure why the term "mostly structurally normal heart " is followed by a "dilated right ventricle". Depending on the expertise of the reader and the site where the right ventricle was measured, 3 cm diameter is normal. The "total load" of 20 PVCs/day is not alarming in a structurally normal heart. You need to discuss these findings with your doctor in detail . Good luck.
