For a few months i can hear but can't understand even if the person speaking is near me sometimes.i can hear all other distant sound perfectly.a little pain in ear when open or close jaw along with some sound in jaw.a little cough.should i get worrie?

You describe problems with speech discrimination which maybe secondary to ear wax buildup or hearing loss from other mechanisms. See your doctor and perhaps undergo an audiogram.
