How can i get a doctors excuse?

You contact your doctor. Such excuses are legal documents that must stand up to court scrutiny. Docs who provide them to patients they do not know for issues they have not assessed can have their license restricted or revoked. Your primary care doc is your best resource in coordinating locally such needs. Alternatively, an urgent care facility can be used if no primary care doc available. Confirm with school or employer first type of document and who can issue it for you.
