Such notes become legal documents that must stand up to a court challenge backed by a physician who knows the patient and his/her exam findings. Filling one out without this knowledge can result in sanctions by the medical board.

The only doc that can help with such papers from a legal standpoint will be the one who was aware of your medical issues, has their own or having access to documentation done in a different setting. Ideally your family doc if having one who can coordinate locally.
