Unprotected receptive anal sex is the highest risk for HIV of all common sexual practices. Ejaculation or not probably makes little difference. Depending on the frequency of HIV among men having sex with men in the area where this happens, PEP probably makes sense; see a local doctor or clinic with lots of experience with HIV prevention and PEP and follow that advice. And get in the condom habit!
