Regarding TMJ, splint may help but will take a long time. Consider a second opinion from oral surgeon. The tinnitus would not be caused from jaw issues, and therefore, an otolaryngologist should be consulted.

First increase your occlusal orthotic user to all of the time you are in the house, not just night-time. Seek evaluation by Oral Surgeon to determine if joints are anatomically damaged. Seek evaluation by dentist with advanced training in bento-facial pain management. TMJ will not cause tinnitus, so see ENT.
