How do i retrieve my doctors note that the doctor sent to my chart?

This is not the place to ask. If you consulted a Healthtap doc on a pay site you need to contact the help desk of that site for such questions or go to www.connect@healthtap.com. This separate site features volunteer docs who answer health questions, It does not share information with other HT sites.

You log in to my chart and go to the date of your visit. After going through several things in the list will be either the doctor’s note or visit summary which contains the salient features of the visit. In few institutions, patients have to request the note from medical records and they print it for you. Hope it helps.
