Dr took my bp with my arm up in the air instead of resting in the table, my bp came out high 140/90. the cuff was too little for my arm to the point it hurt. told her but she didn't paid attention. can it be the cause of that reading?

Possibly. Too small a cuff can give you a spuriously high reading. Also the cuff should cover most of the upper arm and your elbow should be at the heart's level with the arm relaxed and supported on a surface and with your feet resting on the floor.
