Hctz of 12.5 mg is appropriate if your kidney function is good but if kidney fx is weak, this dose may not be effective. You have a relatively high resting pulse rate because of Amlodipine adverse effect. I suggest a maximal dose of Lisinopril 40 mg (AM), daily or 20 mg, bid + Hctz 12.5 mg (AM) and Amlodipine 5 mg (PM). If diastolic pressure or heart rate is still high in 4 weeks, ask me again.

Before a third drug is added, I would prefer that your doctor increase the dose of lisinopril. The current dose of lisinopril is small and it can be increased safely. The fewer medications you are on with good control, the better. Hope you will discuss this with your doctor. Good luck.
