In young individuals without evidence of heart disease, second degree AV block with Wenckebach has a benign course during anesthesia and surgery, and no special precautions are necessary. However, this determination should be made only after careful assessment and exclusion of structural heart disease and discontinuation of cardiac depressant drugs. If I were clearing a patient preop who has Wenckebach or Mobitz type 1 second degree heart block who is otherwise quite stable from a medical and cardiac standpoint, I would advise the anesthesiologist to outfit the patient with an external standby pacemaker as general anesthesia has been known to convert 2nd degree heart block to complete heart block only transiently during anesthesia.
