When a swab for a quick antigen test/or throat culture is positive for the germ group A streptococcus. About 1/2 of sore throats are viral and self heal.

You need to be tested for streptococcus. Onsite tests can make the diagnosis, Culture is the definitive method for making the diagnosis.
