What do brown birth marks mean and why does it hurt when i touch it (on my back neck)?

This is hard to answer. There are birthmarks that are patches of pigment in various intensities/shades. There are brown birthmarks composed of raised/hairy skin. There are simple brown moles. Without a full history, exam or more detailed description there is little to offer that to suggest you have this evaluated during a visit.
