A "densely echogenic focus" reported by U/S from an ovarian source can be a solid blood clot or a growth inside the ovary, but does not suggest PCOS. With PCOS the decreased to absent ovulation inherent in PCOS produces very long (weeks to months) irregularity to bleeding episodes 2/2 sustained estrogen production by ovaries.
